Lynch NP Services provides medical documentation when it is clinically appropriate, accurate, and aligned with professional standards. We understand that paperwork can feel stressful or urgent, and our goal is to support you in a way that is both helpful and responsible.
Because medical documentation carries legal and professional implications, not all notes or forms can be completed in all situations. This page outlines what we can help with, what requires a specific appointment, and what is generally outside the scope of our clinic.

Sick Notes (Medical Notes for Work or School)

We can provide sick notes to support absences from work or school when there has been a recent clinical interaction with our clinic and when it is clinically appropriate to do so.
Important details:
  • Sick notes cannot be backdated beyond 14 days
  • Notes are based on clinical assessment and professional judgment
  • Notes confirm functional impact or need for absence; they do not guarantee employer or institutional acceptance
Sick notes are not issued automatically and may not be appropriate in all circumstances.

Documentation for Diagnoses Made at Our Clinic

We can provide documentation for official diagnoses that are made through assessment at Lynch NP Services, such as ADHD or other mental health conditions evaluated within our clinic.
This may include:
  • Diagnostic confirmation letters
  • Summary documentation for employers, schools, or other healthcare providers
  • Supporting documentation aligned with assessment findings
Documentation is limited to conditions we have personally assessed and diagnosed.

Medical Forms & Paperwork Appointments

If you require a form to be completed, a dedicated appointment is required to ensure accuracy and adequate time for review.

Short Forms (1–2 pages)

  • Typically administrative or workplace-related forms
  • Completed in a 15-minute appointment
  • Fee: $49

Longer Forms (More than 2 pages)

  • Forms requiring detailed responses, review of history, or narrative explanations
  • Completed in a 45-minute appointment
  • Fee: $149
Fees reflect time, complexity, and professional responsibility involved in completing documentation.

Important Limitations on Forms We Complete

To protect clients and ensure documentation has the best chance of being accepted, certain forms are not routinely completed by our clinic, especially when there is no long-term therapeutic relationship.
These include, but are not limited to:
  • ODSP and other provincial disability support applications
  • Insurance disability forms
  • WSIB forms
  • Legal, court, immigration, or human rights tribunal forms
  • Fitness-for-duty or safety-sensitive clearance forms
  • Backdated disability, accommodation, or capacity forms
These forms often require longitudinal knowledge, collateral documentation, or ongoing care relationships that are outside the scope of episodic or assessment-based virtual care.

Important Exception: Disability Tax Credit (DTC)

We do complete Disability Tax Credit (DTC) forms for ADHD when:
ADHD has been formally assessed and diagnosed through our clinic
Clinical criteria and functional impact requirements are met
DTC forms are completed through a dedicated appointment and reviewed carefully to ensure accuracy and defensibility.

Extenuating Circumstances & Pre-Booking Review

In extenuating circumstances, we may be able to assist with certain forms listed above. However, this must be discussed before booking.
